Woman of Non-Childbearing Potential Treatment Initiation Form must be completed for each woman of non-childbearing potential prior to the initiation of their lenalidomide treatment. Male Treatment Initiation Form must be completed for each male patient prior to the initiation of their lenalidomide treatment.
